Pneumoconiosis

Pneumoconiosis can be interpreted as the accumulation of dust in the lungs and the tissue’s reaction to its habitation. It is considered to be an occupational lung disease caused by the long-term exposure to dust. There are several interstitial lung diseases caused by the inhalation of certain dusts. The primary cause of the pneumoconioses is work-place exposure; environmental exposures rarely give rise to these diseases. Pneumoconiosis refers to a range of diseases that are caused by the inhalation of a range of organic and non-organic dusts which are then retained in the lungs. The main types of pneumoconiosis are: Asbestosis, Berylliosis , Byssinosis, Coal Worker’s Pneumoconiosis (also known as “black lung”), Kaolin Pneumoconiosis, Siderosis, and Silicosis. The most common type of pneumoconiosis is Coal Worker’s Pneumoconiosis (CWP). The Black lung is actually a set of conditions and it wasn’t till the 1950s that its dangers were not well understood. There are currently about 130,000 underground coal miners that are currently working in the United States. Mining and the production of coal is a large part of the economy in several developed countries therefore, leaving thousands of American miners dead from CWP. Even though this disease is preventable by using the appropriate safety equipment, there are still many miners that end up developing advanced and severe cases of the disease. The coal dust itself is not as fibrogenic as silica dust is. Once the coal dust enters the lungs it can neither be destroyed nor removed by the body. When the coal dust is inhaled it reaches the terminal bronchioles, then is engulfed by alveolar and interstitial macrophages. Surface mining is when the soil and rocks overlying the mineral deposits are removed. It is used when deposits of commercially useful minerals or rocks are found near the surface; that is, where the overburden(surface material covering the valuable deposit) is relatively thin or the material of interest is structurally unsuitable for tunneling (as would usually be the case for sand, cinder, and gravel). Surface mining is safer than underground mining because the miners are not exposed to such potential hazards as roof falls, to explosions caused by methane gas or dust ignitions, and to coal-worker pneumoconiosis (black lung) caused by long-term exposure to respirable coal dust. ...Da Ai Qing Chen(大爱清尘) Origin Wang Keqin, the founder of Da Ai Qing Chen, was a very famous journalist before 2011. In the middle of nineties, he got a chance to interview the coal miners working in Gansu province. At that time, he met the vice-supervisor – Lao Huang, who is responsible for supervising the coal mining, and Lao Huang was having a very serious dust lung (pneumoconiosis). He coughed all the time and sometimes he could not breathe at all. After a few years, Lao Huang died from the disease for not getting any single fresh air. Later, more cases went to Wang Keqin’s desk. Then he realized it is such a severe issue that there must be someone or some groups to help those hopeless people with the disease. Finally, he decided to help those helpless workers and established the charity group – Da Ai Qing Chen. Brief Introduction Dust lung (pneumoconiosis) is the most serious occupational disease in China. According to the statistics from the department of public health, nearly 90% of the occupational disease is dust lungs and the death rate is as high as 22%. Based on the research, in China, there are at least 6 million dust lung patients. Almost all of them are from lowest class. They do not have a penny to spend on hospital so to their data, currently China has at least six million peasants with the disease. Alveolar or interstitial macrophages uptake carbon particles and then accumulate in the CT(Connective tissue) along the lymphatics. Dependant on the gravity of exposure two types of tissue outcomes can develop. The first is associated with a milder outcome and involves the so-called coal macules and nodules. The macules consist majorly of carbon rich macrophages. In Carbon nodules there are additional collagen fibers that can be found. Those lesions are generally next to the respiratory epithelium that can lead to centrilobular emphysema development. The other type, referred to as complicated coal workers pneumoconiosis, is associated with a more severe outcome of disease. Coal dust represents a significant stimulus onto which macrophages react by releasing cytokines, oxygen, reactive oxygen species and fibroblast growth factors. These act as mediators of inflammation and fibrosis leading to the formation of black scars (1-10cm in diameter). They are usually multiples and microscopically seen they consist of collagen and pigment.
