The total amount of electrical power in a vehicle is determined by the capacity of the alternator. The power must serve over twenty subsystems, such as the stereo, the engine, the instrument panel, and so on. These subsystems are developed and controlled by separate “chimney” organizations, and power allocations must be made for each subsystem. The problem was, in this vehicle program, when the requirements of all the chimneys and teams were added up, they equaled 125 percent of the capacity of the alternator. Keith, who had recently taken over as head of this vehicle program (which had made changes in direction and was behind schedule to begin with), called a meeting of the Program Steering Committee designed to resolve this conflict and reach a compromise. However, many of the chimney representatives who were members of the team came to this meeting with instructions from their bosses [who, incidentally, did their performance appraisals] not to make any compromises, but to make certain that their chimney “got what it needed” and “didn’t lose out.” After Keith presented the group with the problem and the need to reach a compromise solution, their response surprised him: “It’s not our problem,” they replied, “it’s your problem.”

1. What advice would you give Keith for dealing with the Program Steering Committee?

I suggest him to modify the rules of compliance with his team and implement more monitoring and mentoring system among them, in order to figure out any problem and who is the responsible of making it.

2. Which other key stakeholders should Keith deal with? What advice would you give him for dealing with these other key stakeholders?

Communication is the key to success, I believe that without a critical understanding a person cannot function or proceed to their daily operations. As an assistant manager at Wal-Mart, I deal with all of the three critical errors. Out of the three critical errors in communication I deal with the most at work has to be the assumption observation error. As an assistant manager, I have to plan out what to do with the items that are coming in on the trucks daily. The items can range from anywhere being hundreds to thousands items per day. I have to plan where to put the items with my other managers and department managers. The easiest way to plan is to get a print out of a truck listing that breaks down the items by departments. Then, we delegate the listing by departments to each individual department manager; for example: I give the electronics items listing to the electronics department manager and he or she gives me feedback on where to put the item. Then we have an afternoon meeting to type what items will go where, when it is unloaded from the truck. The assumption observation error comes into play when the overnight manager uses the information given to him or her on where to put the item. ...My involvement in the drug error is as follows. I was working on the night shift as the only qualified nurse with 2 nursing assistants. The late shift decided to administer the 10pm medications as a way of helping me. This however was key in me making the error that I did. If I had been left to do the 10pm medications by myself, this error would not have occurred. Patient PF was given her medication by the late staff, however she had spat them out. On going to give her these again, I also repeated her liquid medication which she had actually taken with the late shift before she spat out her tablets. PF took half the liquid before giving it back to me saying that she had already taken it and it was just her tablets that she didn’t take. She accepted the tablets no problem from me. I checked the BNF to see limits of medication and knew the extra that she had was well within the maximum dose. I also knew that she was not naive to medication and had been taking this medication for some time without any adverse effects. I checked her observations and BP, pulse and temp were all within normal limits. I continued with getting the other ladies ready for bed and carried on with my regular night time duties and once the ward was settled I filled in the datix form to report the error. ...Drug Errors Introduction Administration of medicines is a key element of nursing care. Every day some 7000 doses of medication are administered in a typical NHS hospital (Audit commission 2002). Drug administration forms a major part of the clinical nurses role. Medicines are prescribed by the doctor and dispensed by the pharmacist but responsibility for correct administration rests with the registered nurse (O'Shea 1999). Each registered nurse is accountable for his/her practice. This practice includes preparing, checking and administering medications, updating knowledge of medications, monitoring the effectiveness of treatment, reporting adverse drug reactions and teaching patients about the drugs that they receive (O'Shea 1999). The patient is expected to receive the correct medication at each drug round but several studies have shown that this is not always the case (Raju et al 1988, Ferner 1995). Medication errors do occur and are a persistent problem associated with nursing practice (O'Shea 1999). The aim of this paper is to answer the question: Why are nurses still continuing to make drug errors? Troubleshooting Plugins in PowerPoint The best way to troubleshoot problems with plugins is to start PowerPoint in safe mode and block the plugins from loading one at a time until the problem plugin is found. To do this you will first load PowerPoint into “safe mode”, this can be done by using the “windows key +R” to bring up the run menu and typing in “powerpnt /safe’. When PowerPoint loads the words (safe mode) will appear in the middle of the title bar. The plugins can be found in the File tab under the Options settings in the Add-Ins category. This will list the plugins that are installed, to disable one click the drop down menu at the bottom of the window and select COM Add-Ins then click the “Go” button, this will bring up a window that will allow you to uncheck plugins you wish to disable.
